2. IMMUNIZATION RECORDS

Each child in attendance at Clubhouse Playcare must meet the immunization requirements specified by the Texas Department of State Health Services. During the registration process, parents will need to provide ONE of the following:

A copy of your child’s current immunization record (example) that includes your child’s name and DOB. If your child is not current on their immunizations for medical reasons, a signed letter from the child’s physician documenting the reasons why will be needed in addition to their immunization record.

For children over 5 years of age, parents can note on their registration that the child’s immunization records or affidavit are on file at the school that the child attends. In this case parents will need to provide the school name and phone number. You will not need to provide any copies during registration.

The signed statement must indicate the child is free of communicable diseases and able to take part in a childcare program. If your child’s physician is unsure of the documentation needed, you can print out a copy of our form and have them fill it out and sign it.

4. EMERGENCY ACTION PLAN

Children with severe food allergies or medical conditions that may need emergency attention are required to have a physician signed Emergency Action Plan (example) on file with Clubhouse Playcare to be enrolled. If the Emergency Action Plan states that prescription medication (such as an epi-pen) needs to be administered, parents must check-in/out medication with the front desk staff for each stay. Medication must have an original pharmacy label displaying the child’s name and correct dosage/application. Children will not be permitted to enter the facility if they do not have their emergency medication.
